YOUR PERSONAL INFORMATION

Mandurah Community Care (Inc) collects personal information during your assessment interview to enable the Agency to deliver efficient services to you.

Your personal information is used to deliver a service and also, with your permission, data is sent quarterly to a Home and Community Care central Agency to aid in the distribution of funds.

The information that is collected will NOT be disclosed to another person or Agency without your permission. You may withdraw your permission at any time. Exceptions to this will be where the Act specifies exemptions apply.

These exemptions may include: a serious and imminent threat to an individual’s life, health or safety, a serious threat to public health or public safety, an unlawful activity has been or is engaged in, criminal offences, the protection of public revenue, coronial inquiries and proceedings before any court.

HOW WE COLLECT PERSONAL INFORMATION

Mandurah Community Care (Inc) collects information in a number of ways;
Directly from you at your assessment interview; Indirectly through family, carers, Aged Care Assessment Team, your doctor; Other community service agencies, with your permission.

HELP US TO ENSURE WE HOLD ACCURATE INFORMATION

Mandurah Community Care (Inc) takes all reasonable precautions to ensure that the personal information the Agency collect, use and disclose is accurate, complete and up to date.
However, the accuracy of that information depends to a large extent on the information you provide.

That’s why we recommend that you:
- Let us know if there are any errors in your personal information
- Keep us up to date with changes to personal information such as name and address.
